Teleconference CD: "Adverse Drug Events: Medication Error or Adverse Drug Reaction" (2/19/2009)

Are you able to determine if an adverse drug event was preventable or not?  Adverse drug events include those that are nonpreventable (adverse drug reactions) and those that are preventable by definition (medication errors). This teleconference provides an overview of classification of adverse drug reactions and focus on the "preventability factor" involved in many of these "nonpreventable" adverse drug events. Case vignettes will demonstrate how many nonpreventable adverse drug events may indeed be a facet of medication errors, primarily those classified as medication errors of omissions.
