Program Overview:

In March of 2011, the Institute for Safe Medication Practices (ISMP) published a Medication Safety Alert! which told the story of a physician who inadvertently entered several medication orders electronically into the wrong patient's record, leading to the patient's respiratory arrest and ultimate demise. Since this alert, new approaches have been developed for identifying and preventing wrong patient orders in electronic health record (EHR) systems. In 2013, the Office of the National Coordinator for Health IT (ONC) published the Patient Identification SAFER Guide which recommends the use of safety practices for reliably identifying patients in EHR systems.

Join ISMP and our speaker Dr. Jason Adelman as he describes the challenges of wrong patient errors in current electronic systems and the use of an automated "retract and reorder" measure to discover wrong patient errors not visible in most error reporting systems.

Participants will learn about the possible causes of these events and as well as the value of applying Just Culture principles when approaching this type of adverse medication event in health systems.
