Program Overview:

This session will explore the basic tenets of a Just Culture and how these tenets can be applied to the investigation of medication safety events and accountability for safety—both system and individual.The speaker will begin with a discussion about outcome-based and rule-based models of accountability and how they contrast with Just Culture—a risk-based model of accountability. Participants will learn to differentiate between human error, at-risk behavior, and reckless behavior, and how to manage each of these behaviors in a manner that maximizes safety and is fair to the workforce. The speaker will close with an overview of the questions and processes most critical to the successful investigation of adverse events, with a particular focus on conducting a human error and at-risk behavior investigation.
